To bridge the gap between care planning and everyday living by providing non-medical, practical, relationship-centered case management that helps adults access resources, overcome barriers, & achieve greater stability to preserve independence.

Our Case Managers transform complex systems into practical solutions, including:

  • Scheduling and coordinating medical, behavioral health, and specialty appointments
  • Transportation planning and appointment follow-through
  • Medicaid, Medicare, SNAP, SSI, SSDI, and other public benefit applications and renewals
  • Housing searches, housing stabilization, and landlord communication
  • Organizing paperwork and important documents
  • Communication between providers, agencies, and support systems
  • Community resource navigation
  • Daily planning, organization, and executive functioning support
  • Goal setting, accountability, and routine development
  • Support with forms, applications, and administrative tasks
  • Advocacy within community systems

Our Scope

LT provides non-medical, community-based Case Management.

We do not provide:

  • Home health or personal care services
  • Medication administration or medication management
  • Skilled nursing
  • Therapy or counseling through this service
  • Legal guardianship
  • Trustee or fiduciary services
  • Financial management
  • Emergency or crisis intervention
  • 24-hour supervision or caregiving

We do not provide therapy, rather we coordinate support based on the individual needs of the client.

All of our case managers hold bachelors or masters degrees and/or equivalent experience in case management or related services.

Each client's needs are unique. Service frequency is determined by the individual's level of support needed, and referral recommendations. Some clients benefit from weekly contact, while others may require more frequent support during periods of transition or increased need. Our goal is to provide the right level of support to promote stability, independence, and meaningful progress.

LT provides concise, regular email updates summarizing completed tasks, current progress, and any identified barriers or next steps. Additional communication is provided as needed for time-sensitive matters or significant changes, ensuring guardians and care teams remain informed without being overwhelmed.

LT accepts payment by credit card, check, and ACH bank draft. We work directly with guardians, trustees, fiduciaries, families, or other authorized payors to establish a billing process that best meets their needs. Please contact us to discuss service rates and billing arrangements.
